Complete online Parent & Student permissions. This should be completed for every sport you plan to participate in for the full school year.
Turn in WIAA Physical Cards to the Athletic Office. These can be emailed to email@example.com or mailed to: Madison West High School, 30 Ash Street, Madison, WI 53726 Everyone who participates in interscholastic athletics, cheerleading, or pom pon must have a physical examination the first year out and then on alternating years thereafter. This examination must be attested to with the signature of a licensed physician or nurse practitioner or physician’s assistant on the green WIAA physical examination card.
"Green" Card- Physical examination card- must be signed and stamped by licensed physician/nurse or practitioner/physician's assistant and parent (both sides must be complete).

Athletic Fees: Athletic participation fees are collected ONLINE for each sport after final rosters are set (fees will show up in Infinite Campus under the Fees tab - same as all other school fees), and are due before the first competition. Fees are $115 per sport [plus surcharge fees for golf ($118), hockey ($800), gymnastics ($100), and wrestling($100)]. All fees are non-refundable after the first date of competition. Athletic fees are separate from school fees and must be paid separately online. A fee waiver request is available for families in need. Tryouts at West generally take place the first week to two weeks of the official start of each season.
Student athletes are given the opportunity to have a minimum of 3 days during a tryout, but some sports, at the coaches discretion, may lengthen the tryouts dependent on number of athletes, available facilities and other related factors.
Student athletes must complete the registration process in order to be eligible to tryout for sports.
Participating in sports at Madison West is a privilege, not a right and must be earned. The tryout process for sports at Madison West is competitive and may force coaches to make roster selections (aka, cuts) for varsity, junior varsity, and freshman teams. Some teams are no cut.
All sports teams that have tryouts provide each student athlete a fair and objective evaluation, minimum of three days and make the best decisions based on the tryout. As much as we want everyone to participate in sports at Madison West, roster selections will be made and some student athletes may not make the sport they are trying out for. Head Coaches will make every effort to explain to each student athlete why they did not make the team.
